Your In-Network Out-of-Pocket Costs
- Routine preventive care: Free
- Primary Care Physician office visits: 10% (no deductible)
- Specialist office Visits and Emergency Room visits: 30% (no deductible)
- All other eligible services: 30% after the deductible
- Deductible: $1,000 for individuals and $2,000 for a family
- Out-of-pocket maximum: $3,000 for an individual or $4,500 for a family
