Employees told us they want to pay less for their medical plan. But, it's tough to design a plan that costs less and provides good coverage.
This plan does that. It offers coverage without a deductible for many services, including routine in-network preventive care, office visits, and emergency room care. And, it covers everything that the more expensive plan does.
What You Need to Know
- If it's covered today, it's covered under this plan.
- This is a good plan for healthier people or those who get most of their care from a primary care physician.
- This is the least expensive medical option from your paycheck, but it may cost you more when you use medical services.
Your In-Network Out-of-Pocket Costs
- Routine preventive care: Free
- Primary Care Physician office visits: 10% (no deductible)
- Specialist office Visits and Emergency Room visits: 30% (no deductible)
- All other eligible services: 30% after the deductible
- Deductible: $1,000 for individuals and $2,000 for a family
- Out-of-pocket maximum: $3,000 for an individual or $4,500 for a family
