This program limits quantity and dosing levels to encourage pharmacists and doctors to adhere to the quantity recommended by the National Prescribing Guidelines and the Federal Drug Administration (FDA).

Your retail pharmacist will receive a message on his or her computer system if you attempt to fill a prescription for a quantity above the established limit.

To get approval for a quantity above the established limit, either you or your pharmacist must contact your doctor. Your doctor must then contact CVS Caremark's Prior Authorization Department at 1-888-413-2723.

If your doctor determines that you need a greater quantity of drugs than the quantity approved by the claims administrator, you can obtain those amounts, but at your own expense. They will not be covered by the plan.
