Your Vision Benefits
Choices includes one vision plan. Vision benefits under the plan cover routine vision exams, lenses or contacts, and frames for you and your covered dependents. A licensed ophthalmologist, optometrist, or optician must provide the services. See the coverage chart for details.
In-Network or Out-of-Network?
Davis Vision, Alcoa's vision claims administrator, has an extensive network of providers who accept negotiated or discounted rates. You can choose an in-network or out-of-network provider. Here are some differences you need to know:
- If you use an in-network provider, your plan covers a larger portion of your expenses and you have no claim forms to file.
- If you use an out-of-network provider, you must pay the provider directly and then submit a claim to the vision care claims administrator. You will be reimbursed for covered vision services up to the out-of-network benefit limit. Click here for a claim form.

How Can I Pay for Vision Services?
You are responsible for a deductible and expenses not covered by the plan. Ask your provider what you must pay at the time of service and what will be billed.
For convenience and savings, you can you the following Alcoa accounts. Because flexible spending accounts have the "use it or lose it" rule, use these funds first when you have vision expenses.
- Health Care Fund flexible spending account
- limited flexible spending account
- Health Reimbursement Arrangement (HRA)
- Health Savings Account (HSA)
What Expenses are Covered?
Covered vision expenses include:
- routine eye exam once per calendar year
- standard lenses or contact lenses once per calendar year (more may be covered if your prescription changes a lot in one year)
- frames once every two calendar years
- contact lens fitting once per calendar year
Is Preventive Care Free?
Yes. When you use an in-network provider, your annual routine vision exam is free.
What Happens If I Hurt My Eye?
Generally, treatment for disease or injury to the eye is covered under your medical
benefits coverage. Call your medical doctor or, in the case of a serious injury, go to your local hospital emergency room.
What Happens if I Need Vision Care When Traveling?
Call 1-800-433-9906. A Davis Vision customer service representative may be able to give you names of in-network providers in the area that you are visiting. As is true when you are at home, your best bet is to use an in-network provider if possible.
