Q: What is the coverage for dental implants?

Coverage for dental implants is only available in the Comprehensive Dental plan and implants will be covered under the "Major Restorative" category. So, coverage is 50% after deductible ($25/person-$50/family) up to the $2,000 annual per person maximum. There are two standard coverage limitations that will apply to dental implants:

  • Implants will only be covered for participants age 18 and over.
  • An implant will only be covered once per tooth per lifetime.

Remember, whenever you or a covered dependent expects to have a dental treatment that may cost more than $300, you should request a "pre-treatment estimate" (also called a predetermination of benefits) from United Concordia. This will let you and your dentist know in advance how the treatment will be covered and also point out any possible alternative methods of treatment that may be less expensive.
