Basic Plan

 

The Basic plan costs less and provides good coverage. It offers coverage without a deductible for many services, including routine in-network preventive care, office visits, and emergency room care. And, it covers everything that the more expensive plan does.

 

What You Need to Know 

  • This is a good plan for healthier people or those who get most of their care from a primary care physician.
  • This medical option costs the least in payroll deductions, but it may cost you more when you use medical services.


Your In-Network Out-of-Pocket Costs

  • Routine preventive care: Free
  • Primary Care Physician office visits: 10% (no deductible)
  • Specialist office Visits and Emergency Room visits: 30% (no deductible)
  • All other eligible services: 30% after the deductible
  • Deductible: $1,000 for individuals and $2,000 for a family
  • Out-of-pocket maximum: $3,000 for an individual or $4,500 for a family
